Each camper must fill out a registration form. Upon completion of your form you will be taken to the payment page to submit your deposit.

CAMP RULES *

I HAVE READ AND AGREE TO THE ABIDE BY ALL CAMP RULES

WELLNESS POLICIES - I HAVE READ AND AGREE TO THE FOLLOWING *

My dog is healthy and free of parasites. My dog is current with state required vaccinations. (Puppies may not be and are not required to be fully vaccinated due to age and veterinary recommendations). I will not bring my dog if he/she is showing any signs of illness (including coughing) parasites or fleas. BITCHES IN SEASON may not attend. HUMAN HEALTH - In the event I am not feeling well or exhibiting any symptoms of illness I agree not to attend. I will notify Sumac if this occurs.

AGGRESSION POLICY - I HAVE READ AND AGREE TO THE FOLLOWING *

MY DOG HAS NO KNOWN HISTORY OF AGGRESSION. THIS INCLUDES ANY INCIDENTS OF NIPPING, GROWING, SHOWING OF TEETH, LUNCHING OR INTENT TO DO HARM. (Puppies under 5 months of age may exhibit mouthing and playful grabbing and growling and is not considered aggression) Please contact Sumac prior to registering if this is in question. I UNDERSTAND THAT FOR THE SAFETY OF ALL - IN THE EVENT MY DOG EXHIBITS AGGRESSIVE BEHAVIOR WHILE ON THE GROUNDS HE/SHE MUST BE REMOVED FROM THE GROUNDS IMMEDIATELY AND MAY NOT RETURN.

PHOTO RELEASE *

Photos and or videos may be taken at camp. Wag It Inc reserves the right to use these photos and videos in any format. I (we) hereby grant Wag It Inc., their representatives, agents, and assigns, all rights and permissions to use or appropriate my dogs and my name, biography, video or still images and or voice, for reproduction or broadcast in any format or media now known or hereafter to become known. I (we) hereby release Wag It Inc. & Sumac Grant-Johnson, their representatives, agents, and assigns from any claim or cause of action for invasion of the rights of privacy, right of personality, or any similar right.

RELEASE - I HAVE READ AND AGREE TO THE FOLLOWING *

I understand dog training is not without risk. I (we) agree to hold Wag It, Sumac Grant-Johnson, Brian Johnson, camp staff, agents, any employees or volunteers of the aforementioned parties and Camp Marist, harmless from any claim or loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of the dog or handler while on the grounds. I (we) personally assume all responsibility and liability for any such claim. I (we) further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, death or otherwise, and from any claim for damage or injury to the dog, whether such loss, disappearance, appearance, theft, damage, or injury be caused or alleged to be caused by the negligence of the parties aforementioned, or by the negligence of any other person, or any other cause or causes. I (we) hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death resulting at any time there from, sustained by any person or persons, including myself (ourselves), or on account of damage to property arising out of or in consequence of my (our) participation in this event, howsoever such injuries, death, or damage to property may be caused, and whether or not the same may have been caused or may have been alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents or any other persons.

REFUND POLICY - I HAVE READ AND AGREE TO THE FOLLOWING *

I understand BOTH DEPOSITS AND BALANCE PAYMENTS are non refundable.
